Adding a client feedback form in WordPress can be done using several plugins & WP Forms is among one them. Here are the steps to add a client feedback form using the WP Forms plugin:
- Install and activate the WPForms plugin in your WordPress dashboard.
- Go to WPForms > Add New to create a new form.
- WPForms has pre-built templates that you can use as a starting point for your feedback form.
- Customize your form by adding and editing fields. You may want to include fields such as Name, Email, Rating, and Comments.
- Once you think the form is ready, click Save.
- Go to Pages > Add New to create a new page for your feedback form.
- Give your page a title and add the WPForms block to the page.
- Choose the feedback form you created in step 4.
- Publish your page.
Your client feedback form is now live on your website. Clients can submit feedback by filling out the form, and their responses will be emailed to you. You can also view feedback submissions in your WordPress dashboard under WPForms > Entries.

Open your Google Form and click the SEND button at the top right corner. Send form popup screen will be displayed. Click on <> embed option and copy the HTML code. Open your WordPress editor for the page where you want to embed the form, switch to the TEXT mode and paste this HTML code. Save and publish the page to make your form live.
